All incoming students are required to use the Student Health Portal to submit medical immunization records. 提交这些信息很容易!
There is NO fee associated with submitting your records, which gives you lifetime access to your uploaded documents.
For step-by-step instructions on managing your new Immunization account, follow these steps below:
- 访问 myhealth.vbl-design.com and then login with your C-N credentials and confirm your date of birth. You will see a section on the left labeled “Forms and Registration”
- If you get an error message when to trying to login — note that it may take up to 48 hours for your record to be created after you have been accepted to C-N. Please contact the C-N 健康中心 with any questions.
- 一旦你点击表单和注册选项卡, it will take you to a list of immunizations that you need to be compliant.
- Only the immunizations that are pertinent to you and your degree path will show on this screen. You will click the green update button and it will allow a place to put the dates of your immunization and the type of immunization. You may then upload supporting documentation under the “Immunization Records” tab.
- Your documents will then be reviewed to make sure the dates match and supporting documentation is official. Once approved, it will then say “Compliant and Satisfied”
- All immunization requirements must be met before a registration hold is lifted

- 水痘(水痘) – (2 doses or credible history of disease): if born on or after January 1, 1980, only.

- 脑膜炎球菌 – At a minimum of 1 dose given at 16 years of age or greater if under 22 years of age and living in on-campus housing.
- TB – All students must show evidence of having either a “Low Risk” for carrying TB by filling out the “Low Risk” questionnaire, or proof of no disease by negative TB skin test (<15mm) or Negative Chest X-Ray. The “Low Risk” assessment questionnaire can be found when you log onto your Student Health Portal account.
Minimum ages or dose intervals – Tennessee follows published CDC guidelines. For vaccines with critical minimum age requirements (e.g., MMR, 水痘)或最小剂量间隔, doses are considered valid if given up to 4 days before the minimum age or dose interval. Doses administered more than 4 days early are considered invalid and should be repeated as recommended.
Alternative proof of immunity for certain diseases – A positive serology (year of test documented) is acceptable as an alternative to immunization for measles, 流行性腮腺炎, 风疹, 乙型肝炎或水痘. 为水痘, documentation of provider-diagnosed varicella (Chicken Pox) or provider-verified credible history of disease given by a parent or guardian also is acceptable. 通过记录病史, the provider is asserting that he or she is convinced that the child has had chickenpox.
All students must show evidence of tuberculosis skin screening or complete a Low-Risk TB assessment which is available to fill out when you log on to your Student Health Portal account.

是的, students can request a religious or medical exemption to the immunization requirement. Medical exemptions require the signature of a health care provider.

On the left, you will see a tab that says, “Downloadable Forms”. Click here and you will see a form that says, “Religious or Medical Immunization Exemption”. 下载此表格, 把这个填好, and have it signed by an appropriate medical professional (if applicable), then upload this under the “Immunization Records” tab under Forms and Requirements.
